One of the most common mistakes in sports rehabilitation is returning to play based on time rather than readiness. Research is clear: athletes who return before meeting objective strength, movement, and performance criteria have dramatically higher re-injury rates — particularly for ACL tears, which have a re-injury rate exceeding 20% in athletes who return too early.
At Ageless Athlete Physical Therapy, return-to-sport is not a date on a calendar. It is a series of milestones your body must meet — and we don't clear you until you're truly ready.
Our Return-to-Sport Process
Phase 1 — Tissue Healing & Pain Control
The foundation of all return-to-sport work. We manage swelling and pain, protect healing tissue, and begin early range of motion and muscle activation work to prevent deconditioning.
Phase 2 — Strength & Movement Restoration
Progressive loading to rebuild strength, address muscle imbalances, and restore normal movement patterns. Strength symmetry between sides is a key benchmark before advancing.
Phase 3 — Neuromuscular Control
Balance, proprioception, and reactive training to retrain the nervous system's ability to stabilize joints under dynamic conditions — essential for safe sport participation.
Phase 4 — Sport-Specific Training
Running, cutting, jumping, throwing, or sport-specific drills progressively reintroduced under controlled conditions. Load is managed carefully to avoid setbacks.
Phase 5 — Full Practice & Competition
Graduated return to team practice and full competition. We work with coaches to manage load during this transition and ensure the athlete is monitored appropriately.
Criteria-Based Clearance
Objective tests — limb symmetry index, hop tests, strength testing, sport-specific performance measures — determine readiness. We don't clear athletes based on how they feel alone.
